Care Team Model: 23 CRNA; 7 MD
Length of Need: 3 Months
Avg Time to Credential: 90 days; Temp Privileges with Clean File
Types of Cases: Vascluar, Gyn, Ortho, Trauma, Cysto, ENT, Neuro, Orthospine, Gen, Robotic, Cardiothoracic (Hearts & Valves), Thoracic-Lungs, OB-Non-high risk, GI, EP, MRI, CT, Cath lab Off-Site Locations: ASC's, Cath Labs, Endo-No
Availability
o 1st possible date they would be available?
o How many shifts would they be able for per month?
o What month is their schedule completely open?
o Any shift preferences (Day, Night, Blocks…etc)?
o Any dates in the next 3-6 months they can’t work?
o Any Holidays in the next 3-6 months they would be open to work. ?
