Position Overview
As an Advanced Practice Provider placed through Momentum HCS, you will play a key role in delivering comprehensive, evidence-based care within your specialty. You will collaborate closely with physicians and multidisciplinary teams to ensure safe, effective, and patient-centered treatment.
Key Responsibilities
Provide high-quality, patient-centered care within your specialty.
Perform comprehensive assessments, diagnosis, and treatment planning.
Collaborate with physicians and multidisciplinary healthcare teams.
Assist in procedures and specialty-specific clinical interventions, as applicable.
Interpret diagnostic tests and support clinical decision-making.
Educate patients and families regarding treatment plans and ongoing care.
Maintain accurate and timely medical documentation.
Adhere to clinical guidelines, safety standards, and regulatory requirements.
Qualifications
Master’s or Doctoral degree in Nursing, Physician Assistant Studies, Anesthesiologist Assistant program, or Audiology (as applicable).
Active and unrestricted Vermont licensure or eligibility for licensure.
National certification appropriate to specialty (NP, PA, CRNA, AA, or Audiology certification).
Prior specialty experience required for Cancer Center APP roles.
Strong clinical judgment, communication, and teamwork skills.
