• Board Certified NP/PA - REQUIRED
• Please include address at time of presentation - REQUIRED
• Provider must be local - REQUIRED, no exceptions
• Must have clean background for quick credentialing - REQUIRED no exceptions
• Please provide availability based of dates listed above- REQUIRED
• DEA - Verbal Confirmation Required at Name Clear
• MI License - Required at time of clear, no exceptions - not accepting IMLC
• ACLS/ATLS - REQUIRED
• ACLS (AHA)
• ATLS
• Board Certified
• DEA
