• BC or BE in Emergency Medicine
• ATLS, ACLS & PALS are required for provider not Board Cert in EM & for those holding lifetime BC that are not renewable & not voluntarily re-certified.
• Providers must be able to demonstrate that they have had active practice for at least 3 years in an EM dept with census equal to or exceeding 20,000 patient visits annually (which may include Residency training)
• Moderate Sedation and Central Line placement test (AIM for ZERO)-is required for all physicians
• Minimum privileges for ED providers on DOP
• Shifts: 6a-4p, 7a-5p, 10a-8p, 4p-2a, 8p-6a
• No call
• Core privileges
• Admission privileges: Observation only
• Moderate Sedation
• Central line insertions
• Minimum TWO years experience in EM
• Clinical Interview required

• Must have TX license and DEA at time of presentation
• Must have clean malpractice/background
• Must be Board Certified EM. Will accept Board Eligible within 5 years but 3 years or more must have a detailed explanation as to why the provider is are not boarded yet. EM BE, will need ATLS, ACLS & PALS
• Must send EPA
• Must send self-query NPDB if answer yes on EPA
• Must submit vaccination card with presentation
