SUBMISSION REQUIREMENTS
• BC/BE in Emergency Medicine, Will also take Family Practice with extensive Experience in Emergency Medicine.
• Internal Medicine Trained Physicians cannot work at this location as the state of IL no longer provides EDAP Waiver.
• BE must have ACLS-PALS-ATLS
• Non EM Trained/Boarded providers must have a minimum of 2000 hours of EM Experience, including 30 documented cases of Trauma and 30 of Cardiac Care.
• Case Logs for the past 24 months
