Job ID: FNP Needed in Shreveport, LA
Start date March 16th or as soon as credentialed
3-Months with the option to extend
Position Highlights:
Full-time only (40 hours/week) Monday–Friday
8 am-5 pm with a 1-hour lunch
One-hour lunch break included
Equipment will be mailed out and provided by the client
Telehealth (Home Health)...If providers live close to the patient, it is a possibility they might schedule an in-home care visit, but will most likely be telehealth
MUST BE COMFORTABLE SEEING PEDIATRICS AND ADULTS
Schedules are released 2–3 weeks in advance
Paid for a full day regardless of patient volume (providers must enter availability into Salesforce to be paid for the full day)
1–3 hours of paid orientation, all additional ongoing education modules are expected to be completed during downtime within working hours
Must be enrolled/Credentialed in Medicard (please highlight in submission and confirm with provider)
Role Overview:
Conduct 5–6 home health assessments per day (Possibly will see in-home patients if they live close to the county)
Perform annual risk assessments and chronic condition reviews
Assessment-only role — no treatment or prescribing required
Escalate urgent findings to the patient’s PCP
Must have an active LA Medical license (Must live in the state of Louisiana)
