· Amount of coverage needed per month: Scheduled Clinical Hours Only
· Patients per day: 23

Practice Setting:
• Outpatient
Requirements:
~ Board Certification: No
~ License required: Active NM License
~ Certifications required: BLS, PALS, DO
This is a FQHC and the provider needs to be able to see all ages.
Credentialing Timeframe: 30 Days
