Clinicians will provide outpatient primary care services to patients of all ages at a Federally Qualified Health Center (FQHC), working Monday through Friday from 9:00 AM to 5:00 PM with an expected caseload of 18-22 patients per day. Clinical responsibilities encompass general family medicine services, including acute and chronic care management for a diverse patient population across all age groups. Candidates must hold an active New York nursing license and be enrolled in Medicaid; prescriptive authority is required for this position. FQHC experience is preferred.
