FNP Needed in Barlow, OR (70% telehealth)

Job description

I have a client in OR who's seeking several experienced Family Medicine Nurse Practitioners for locum coverage. Here are the job details.

Start: March

13-week assignment with the option to extend

Monday-Friday

8 am- 5 pm (1 hour lunch)

Full time 40 hours a week or Part time 20 hours a week

Oregon (OR)
Lane, Multnomah & Washington (Please list in the submission which county your provider is interested in).

Contract Length: 3 months with the option to extend

Position Highlights:

70% Telehealth 30% in Home (Home Health)...If providers live in the county, scheduling will try and schdule in home visits, but telehealth will be a main compent
One-hour lunch break included
Schedules are released 2–3 weeks in advance
Open to full-time or part-time providers
Paid for a full day regardless of patient volume (providers must enter availability into Salesforce to be paid for the full day)
1–3 hours of paid orientation, all additional ongoing education modules are expected to be completed during downtime within working hours
Must be enrolled/Credentialed in Medicard (please highlight in submission and confirm with provider)

Role Overview:
Conduct 5–6 in-home health assessments per day
Perform annual risk assessments and chronic condition reviews
Assessment-only role — no treatment or prescribing required
Escalate urgent findings to the patient’s PCP
A1C testing only (if applicable; no additional screenings required)

Travel.
Mileage reimbursed at the IRS standard rate
Must have an active OR Medical License
