Family Practice NP/PA Needed in Phoenix, AZ
Start Date: April 2026
Contract Length: 3 months ongoing
Schedule: Full time, Mon-Fri
Work Details:
• Support staff: MA, RN, care coordinators, behavioral health, front desk
• Patient population/age: all ages
• Key Accountabilities:
o Provides comprehensive primary patient care including prevention, health maintenance, diagnosis, treatment of acute and chronic conditions, and follow-up services to patients under his/her care within the scope of practice of family medicine.
o Documents all patient-provider interactions following health care standards.
o Participates as an active member of an integrated patient centered team.
o Supports the organization as needed.
Practice Setting:
• Clinic/Private Practice
Requirements:
• Board Certification
• DEA: Required
• Certifications Preferred: ACLS preferred and BLS required
• Minimum experience required: Three years of experience in an outpatient family practice or comparable setting is preferred. Experience in a community health center setting is preferred.

Credentialing Timeframe: ASAP
Active AZ License
Medical malpractice insurance, travel, and lodging provided
