Primary Care NP/PA or Family Medicine Physician Needed – Phoenix, AZ | FQHC Experience Preferred

A well-supported community health organization in Phoenix, Arizona is seeking a full-time Nurse Practitioner or Physician Assistant.

This opportunity offers a collaborative clinical environment, excellent support staff ratios, and the chance to work across two established Phoenix-area outpatient clinics located just minutes apart.

Position Highlights
**Location:** Phoenix, AZ
* **Start Date:** June 1, 2026
* **Assignment Length:** 3–6 Months (6-month commitment preferred)
* **Schedule:** Monday – Friday | 8:00 AM – 5:00 PM
*(Providers will rotate between locations — approximately 10 minutes apart)*
* **Patient Volume:** Approximately 17–19 patients per day
* **EMR:** eClinicalWorks (ECW)
* **Credentialing Timeframe:** Approximately 2 weeks
* **Interview Process:** One Zoom interview

Provider Requirements
* NP, PA accepted
* Family Medicine background preferred
* Must be comfortable seeing patients ages 5+
* **Experience Requirements:**

* NP/PA: Minimum 2 years required
* FQHC experience is highly preferred
* Bilingual providers are a plus, but not required

Practice Environment
* Excellent **2:1 support staff ratio**
* Collaborative outpatient setting
* Established patient population and clinic workflows
