Opportunity:
Advanced Practitioners - Primary Care
Job Details:
- Full-time Nurse Practitioner (FNP, ANP, AGNP) or Physician Assistant position within the REACH (Resource for Elderly and Care at Home) Program
- Deliver compassionate primary care in the comfort of geriatric patients' homes and assisted living facilities
- Work within a multi-disciplinary geriatric medicine/primary care team in a home-based setting
- Full-time schedule, weekdays only
- Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients
- Develop plans for further evaluation of health needs, including referrals to other health team members or specialists
- Provide appropriate treatment for problems within the scope of practice and refer appropriately when needed
- Utilize EPIC for state-of-the-art clinical and informatics technology to promote improved patient outcomes and streamlined continuity of care
- Unrestricted State of Connecticut Medical License required (or eligible)
- Minimum of two years of post-graduate experience as an APRN or PA required; Primary Care/Geriatric Medicine experience highly preferred
- New graduates with relevant nursing experience are encouraged to apply
