Tele/Home Health Nurse Practitioner – Louisiana
Start: September 2026
Length: 2-month contract with option to extend
Employment: 1099 Locum Tenens
Schedule: Full-time (40 hours/week, Sunday–Saturday)
Hours: 8:00 AM–5:00 PM with a 1-hour lunch
Role Highlights
Primarily telehealth home health assessments
May occasionally complete in-home visits if the patient is nearby
Conduct 5–6 assessments per day
Annual risk assessments and chronic condition reviews
Assessment-only role – no prescribing or treatment required
Escalate urgent findings to the patient's PCP
Equipment provided by the client
Schedules released 2–3 weeks in advance
Requirements
Board Certified Family Nurse Practitioner
Active Louisiana NP license
Must be credentialed/enrolled with Louisiana Medicaid
Comfortable seeing both pediatric and adult patients

Paid per assessment
30-day credentialing timeline
Paid orientation (up to 4 hours)
Additional education modules completed during downtime while on the clock
