• FQHC located near Brentwood, Long Island
• NP / PA
• July 19 - August 11 (possibly longer)
• MUST HAVE SUBOXONE WAIVER
• All ages preferred, but will take ADULTS ONLY
• M-F 9:00am-5:00pm
• 18-22 patients per day
• Standard outpatient primary care-no procedures
• EMR: eClinicalWorks
• Mileage/Parking and/or Public Transportation reimbursed
