• Family Physician or Family Nurse Practitioner
• Must be comfortable seeing ALL ages
• Provider retiring at the end of the month
• Monday-Friday 8a-5p
• Will accept a provider willing to do a minimum of 3 days/week
• Basic primary care visits, sick/well, check-ups, etc.
• Approx 10-15 patients per day
• Support Staff: 1:1 MA to assist
• EMR: ECW
• No Call
• No T&L
• Provider MUST HAVE Medicare/Medicaid
