Position Overview
Greenlife Healthcare Staffing is seeking a Family Nurse Practitioner (FNP) for an opening with a Community Health and Wellness Clinic in Brooklyn, New York. This position provides comprehensive primary care to patients of all ages in a collaborative, patient-centered outpatient setting.

Qualifications
✓ Education: Master's degree in Nursing from an accredited program.
✓ Licensure: Active New York State Nurse Practitioner license, CME, and DEA license.
✓ Certification: Board Certified as a Family Nurse Practitioner (FNP).
✓ Experience: Strongly preferred 4–5 years of experience as a Nurse Practitioner.
✓ Language: Bilingual Spanish is a plus.
✓ Technical Skills: Proficient in performing physical examinations, ordering and interpreting diagnostic tests, prescribing medications, and utilizing electronic health records.
✓ Soft Skills: Excellent communication and interpersonal skills, strong clinical judgment, empathy, patient-centered focus, and the ability to work collaboratively within a multidisciplinary team.

Key Responsibilities
✓ Perform comprehensive physical examinations and patient assessments for individuals and families across the lifespan.
✓ Record and analyze patients' medical histories and symptoms.
✓ Develop and implement individualized patient care plans and contribute to existing plans.
✓ Order, administer, and analyze diagnostic tests and laboratory results.
✓ Diagnose and manage acute and chronic conditions, including hypertension, diabetes, respiratory infections, and other common illnesses.
✓ Prescribe medications and treatments as appropriate, including controlled substances.
✓ Educate patients and families on disease prevention, lifestyle modifications, medication adherence, and treatment options.
✓ Provide preventive care, including immunizations, health screenings, and routine check-ups.
✓ Collaborate with physicians and other healthcare professionals to ensure coordinated, whole-person care.
✓ Maintain accurate, thorough, and timely documentation in electronic medical records.
✓ Provide compassionate, patient-centered care to a diverse patient population.
