Home Health NP Needed in New York:
Start Date: 11/2026 or As soon as credentialed
Contract Length: 13 weeks (flexible)
Schedule: 7 Days, Flexible scheduling, Monday–Sunday, between 8:00 AM and 8:00 PM
Job Details: Bread and butter cases
- Conduct prescheduled in-home health assessments
- Review medical history and current medications.
- Perform physical evaluations as appropriate to the assessment protocol.
- Document clinical findings, health conditions, and care gaps
- Complete patient-specific forms and required electronic documentation
- This is an assessment-only role
New York State License Required (We can help license you!)
Credentialing Timeframe: 15–30 days, potentially sooner
Medical malpractice insurance, travel, and lodging provided
