✅ Key Responsibilities
✔️ Deliver comprehensive primary care including exams, diagnosis, and treatment
✔️ Manage individualized care plans and coordinate referrals
✔️ Collaborate within an integrated care model alongside behavioral health teams
✔️ Prescribe medications in accordance with Ohio law and FNP practice scope
✔️ Educate patients on preventive care and chronic disease management
✔️ Participate in quality improvement initiatives and care team collaboration
✅ Qualifications
✔️ Education: Graduate of an accredited School of Advanced Practice Nursing
✔️ Licensure: Active Ohio Nurse Practitioner License
✔️ Certification: Board Certified as a Family Nurse Practitioner
✔️ Experience: Minimum 3 years of clinical experience as an NP
Experience in community health, ambulatory care, or FQHC settings preferred
✔️ Soft Skills: Compassionate care, strong collaboration, patient education, and a focus on quality improvement
