• Occupational experience is required
• 7-13 patients per day
• Must be able to see ages 2+
• Mon-Fri
• Schedule:
o Monday 6a-3p
o Tuesday: 9a-7p
o Wednesday: 6a-3p
o Thursday : 9a-7p
o Friday: 6a-12p
• Provider is required to take 30 min lunch
• T&L is included

  • Occupational experience required
  • 7-13 patients per day
  • Travel and lodging covered
