-Manage chronic illness in elderly population, patient education about disease process and plan of care adjustments
-Initial assessment including functional eval, social and safety assessments, cognitive screening and mental health assessments
-Start treatment in the home at the point of care
-Collaborate with patients' PCP and specialists as needed
-RI Active License and DEA
- Need Access to reliable transportation
