Schedule: Flexible, Monday–Sunday, 7a–7p
Call Details: No call (no nights or weekends required)
Practice Details
Conduct pre‑scheduled in‑home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members
Visits include:
Comprehensive medication review
Medical history review
Physical evaluation
No prescribing, no ordering labs, no imaging, and no changes to treatment plans
Average visits per day: 6–9
Evaluations are supported by pre‑populated, patient‑specific forms
EMR: TBD
Requirements
Board Certification: Required
License: Active Tennessee license required (not willing to license)
Education: Master’s degree required
Credentialing Timeframe: 7 days to 2–3 weeks
