Family Practice, General Practice, Geriatric Medicine - FP, Geriatric Medicine - IM
Overview:
Conduct pre scheduled In-home Health Evaluations for Medicare Advantage, Medicaid, and commercial plan members. These visits include a medication and medical history review and a physical evaluation performed in a health plan member’s home. They will be provided a pre-populated, patient-specific form for each evaluation and do not prescribe medicine, order lab tests, or alter people’s current treatment regimen.

Schedule:
• Flexible scheduling options available: Full-Time (FT), Part-Time (PT), or PRN
• Operating hours: Monday–Sunday, 7a-7p
• No nights or weekends required - Providers may choose preferred 8 hour shifts.
Travel Logistics:
• Daily gas allowance provided
• Travel accommodation (if eligible) includes:
o Airfare, Hotel, & Rental Car
• Travel Accommodations Eligibility: Must work a minimum of three (3) full days per week
o A "Full Day" = Availability to complete at least 7 evaluations per day (average visits per day: 6-9)
Important Notes:
• Providers must be comfortable entering homes where:
o Smoking may occur
o Pets may be present
• No prescribing or diagnosing - DEA is not required
