We are seeking experienced Nurse Practitioners (NPs) and Physician Assistants (PAs) for flexible Home Health Assessment opportunities.

Responsibilities

Conduct pre-scheduled in-home health evaluations for Medicare Advantage, Medicaid, and select commercial health plan members.

Visits include:

Comprehensive medical and medication history review
Physical health assessment
Completion of patient-specific documentation provided prior to each visit

This is a non-treatment role:

No prescribing medications
No ordering labs or diagnostic testing
No modifying existing treatment plans
Schedule & Volume
Flexible scheduling options
Average of 6–9 patient visits per day
Requirements

📌 Active NP or PA license in the state of practice

📌 Board Certification

📌 Medicare/Medicaid eligible

Credentialing
Approximately 30-day credentialing process
