Tele/Home Health Nurse Practitioner Needed in North Carolina
Start Date: As soon as credentialed
Contract Length: 3 months with option to extend
Schedule: Monday–Friday, 8:00 AM–5:00 PM (1-hour lunch)
Hours: Full-time, 40 hours/week
Schedules released: 2–3 weeks in advance

Locations:
• Mecklenburg County
• Wake County
(Please specify which county your provider prefers when submitting.)

Work Model:
• 70% Telehealth
• 30% In-Home Visits
If the provider lives in the county, scheduling will prioritize in-home visits, but telehealth will remain the primary component.

Compensation Details:
• Paid for a full day regardless of patient volume (must enter availability into Salesforce to qualify)
• 1–3 hours of paid orientation
• Ongoing education modules completed during downtime within working hours
• Mileage reimbursed at IRS standard rate

Role Overview:
• Conduct 5–6 in-home health assessments per day
• Perform annual risk assessments and chronic condition reviews
• Assessment-only role (no prescribing or treatment required)
• Escalate urgent findings to the patient’s PCP
• A1C testing only (if applicable)

Requirements:
• Active North Carolina license
• Must be enrolled/credentialed with Medicare (please confirm and highlight in submission)
