Telehealth/Home Health Nurse Practitioner – Oregon
Start Date: July 2026 or as soon as credentialed
Contract Length: Month-to-month
Locations Available:
• Lane County
• Multnomah County
• Washington County

Schedule
• Monday–Friday
• 40 hours per week with a 1-hour lunch break
• Flexible scheduling available, including evenings and weekends
• Schedules released 2–3 weeks in advance
Position Highlights
• 70% Telehealth / 30% Home Health
• Must be comfortable seeing both pediatric and adult patients
• Paid for a full day regardless of patient volume, provided availability is entered into Salesforce
• 1–3 hours of paid orientation
• Ongoing education modules completed during downtime within working hours
Role Overview
• Perform annual risk assessments and chronic condition reviews
• Assessment-only role — no diagnosing, treating, or prescribing
• Escalate urgent findings to the patient's PCP
Expected Patient Volume
• Telehealth only: 12–14 patients per day
• Home visits only: 6–7 patients per day
• Mixed schedule: approximately 10 patients per day
Requirements
• Family Nurse Practitioner (FNP)
• Active Oregon license
• Must be enrolled/credentialed with Medicaid (please highlight in submission)
• Comfortable providing both telehealth and in-home assessments
• Comfortable seeing pediatric and adult patients
