NP needed for Home Health Assessments in West Virginia. Up to $160/assessment

Job description

We are seeking experienced FNP, ANP, GNP, or PA providers to conduct in-home health assessments for patients within West Virginia. This is a flexible, travel-based role providing comprehensive health and wellness evaluations in patients’ homes.

Key Details:

Practice Setting: In-Home Health Assessments

Worksite: Traveling to patients’ homes within a designated state

Schedule: Monday through Sunday, 7:00 AM – 7:00 PM

Availability: Minimum of 2 full days per week (does not have to be consecutive; at least one weekday required—weekend-only providers not accepted)

Assessments: 5 to 8 per day, each lasting approximately 45–75 minutes; last appointment typically scheduled at 6:00 PM

Patient Population: Primarily geriatrics

Typical Responsibilities:

Conduct health and wellness assessments for insurance plan members

Perform H&P evaluations

Review medications and dosages

Take vitals including blood pressure, finger stick glucose

Additional Details:

Compensation is based on the number of assessments completed monthly

Providers with higher assessment volume have potential to earn more per assessment

Incentive programs and travel support (car rentals, flights, hotels) available for providers meeting certain volume and scheduling requirements
