Requirements
✓ Active NYS Registered Nurse license
✓ Bachelor’s degree in Nursing required
✓ 2+ years in utilization review, medical review, fraud investigation, or similar; OR
✓ 3 years of clinical/administrative nursing experience
✓ 1+ year of nursing home survey experience
✓ Valid driver’s license & ability to travel for on-site visits
✓ SMQT certification preferred (required within 12 months)
✓ Strong analytical, communication, and documentation skills
✓ Comfortable using digital tools and databases

Responsibilities
✓ Conduct surveys and investigate complaints at long-term care facilities
✓ Evaluate allegations of abuse, neglect, and regulatory noncompliance
✓ Draft Statements of Deficiencies (SOD) using Principles of Documentation
✓ Assess compliance with Plans of Correction and monitor corrective actions
✓ Input findings into state and federal databases
✓ Attend administrative hearings and testify when necessary
✓ Adhere to Department of Health policies and travel to assigned facilities
