Signify Health is looking for Providers who are interested in independently contracting with our company to perform non-invasive, in-home health risk assessments insured under the Medicare Advantage plans that we represent nationwide.

$80 per completed assessment (5-10 member visits per day depending on how you structure your daily schedule) as a 1099 Independent Contractor + a Daily Availability Incentive
While working with us, Signify Health will cover your Medical Malpractice Insurance
Flexible, month-to-month scheduling determined by your availability versus the available visits requested by our health plans for their members.
Mondays-Sundays with no required call, nights, or holidays
Pre-scheduled appointments (we will provide you with a pre-populated, patient-specific form for each evaluation with the demographic information and medical history of the patient as made available by the insurer)
Paid airfare, hotel, rental car, and daily availability incentive when applicable as a Traveler.
When performing assessments, you would not be required to prescribe medicine, order lab tests, do blood work, or alter the member’s current treatment regimen.
Online training will be provided prior to your first day of work.
