Job description

*Specialty: NP/PA Home Health
*Start Date: ASAP
*Contract Length: 13 weeks with option to extend
* Shift & Schedule Details: Create your own schedule, must work 40 hours within a Monday-Saturday window and, 8 hours a day between 8 am – 8 pm.
*Perform annual risk assessments and review of chronic conditions
*No treatment or prescribing – assessment-only role
*Escalate urgent findings to the patient’s PCP
*If applicable, conduct A1C tests only (other screenings left for the patient)
*Travel required; mileage reimbursed daily
*State license required: NJ, must be local
