Physician Assistant Nurse Practitioner Gurnee, IL (40 miles N of Chicago)

Seeking a motivated Physician Assistant or Nurse Practitioner to join our medical practice part time in Gurnee, IL. Our clinic is in Gurnee, IL- approximately 40 miles (55 mins.) north of Chicago.
We are looking for a compassionate provider who enjoys working with a multidisciplinary team helping others, someone who is looking for extra hours or even a possible equity position in the future. We are looking for enthusiastic and caring providers, those who are dependable, have excellent customer service skills and are looking for a long-term commitment. We are a physical medicine office, and the provider will be performing exams, treatments such as joint/trigger point injections, and assisting with patient care. We will train the right candidate if needed.

Duties:
• Patient exam/evaluation (medical history, physical exam, review pertinent records/labs/reports)
• Order, draw and review labs
• Diagnosis and treatment plan formulation
• Patient education to the diagnosis, lab results, treatment care plan options, counseling on health-related topics, prevention, and lifestyle modifications
