FP need for CHC in Harlem, NY
• Harlem
• Patient population is a mixture of:
o Homeless & LGBT
o Substance Users
o HIV & Hep C
• Family Physician is what they want & need, & male is preferred
o They will consider an IM or FNP if they have experience with this patient population
• Board Certified strongly preferred. Will only consider BE if a recent grad
• Must have DEA & already be credentialed with Medicare and Medicaid
• Suboxone waiver a plus, but not mandatory
• March 1st for 3 months+
• Schedule:
o Mon-Wed-Thu: 8:30am-5:00pm
o Tue: 11:00am-7:30pm
o Fri: 8:30am-12:30 (this day is optional)
• Standard primary care- full support staff
• All ages (will consider someone who sees adults only, if they meet all other requirements)
• 75% Adult, 25% Children
• 16 patients per day
• 50% appointments, 50% walk-ins
• EMR: ECW (scribes available)
