: Family Nurse Practitioner
Performance Period: Through December 31st 2023. With potential to extend.
Schedule/Shifts Times:
• 8am to 8pm is clinic open, either scheduled 8 hours or 10 hours depending on schedule.
• Contractor is typically scheduled 11am to 8pm closing shift.
• weekend shifts 4 hours: 1pm to 5 pm.
Call Requirements: None
Patient Ages: All ages
Types of Cases: Outpatient Primary Care
Support Staff:
Scope of Work:
• Average of 16 patients a day.
• Standard Outpatient duties.
• Orientation is 1- 3 days at clinic
Requirements:
• BLS, ACLS
• NPI
• Copy of Current DEA
• Copy of board certificate or letter.
• Good standing with NPDB
• Proof of graduation from accredited school
• 3 references, 2 from peers and 1 from medical supervisor
• CV
• CME for the past 2 years.
Credentialing Timeframe: ~6weeks
